Catastrophe Major Medical Insurance Plan now provides financial protection
against extraordinary medical expenses not covered by your current insurance
or through Medicare up to $2,000,000 in protection, payable for
up to five years, for each accident or illness. These higher maximums
will help ensure that you can afford to receive the medical care you need
when you need it most.
If you have basic medical coverage where you work or have your own coverage,

Easy Acceptance for ATLA
Members
You and your spouse are eligible for coverage provided you are a U.S.
resident, if living in an eligible state, regardless of age, provided
each applicant has not been diagnosed or treated in the last five
years for any of the conditions listed on the application.

Your Choice of Deductibles
You may select a deductible of $25,000 or $50,000. The Plan starts
paying benefits for covered medical expenses after you exceed your
selected deductible (or the amount paid by Medicare or another medical
plan, if greater). You have three full years to satisfy your deductible
for each illness or injury.

How This Plan Protects You
Benefits will be paid after your deductible is met whether paid by
another medical plan, Medicare, or out-of-pocket. The Plan then covers
up to 100% of all eligible expenses* for each covered illness or injury
including:

Convalescent Care

Nursing Home Benefits
No matter what your age, you may need custodial or convalescent care
in a nursing home while you are recovering from a serious illness
or injury. Many medical plans (including Medicare) do not offer this
important benefit.

Home Health Care Benefits
The Plan can also pay reasonable and customary charges for covered
home health care treatment for up to 100 visits in any one calendar
year.
