to the American Cancer Society, one out of every two men and one out of
every three women in the United States will be diagnosed with cancer in
their lifetime.* Fortunately, advances in cancer treatment are saving
more lives than ever before. With these advances in care, however, come
rising health care costs. There are hospital expenses, specialists' fees,
prescription drugs, surgery, day and night nursing care, therapists and
more. Even if your basic health insurance policy covers you for cancer,
you may be required to pay a copayment for medical expenses.

PURPOSE OF THE PLAN

Provides supplemental benefits, paid directly to you, regardless
of any other insurance coverage, for any use you wish.

Can be used to help pay for some of the best treatment available
to fight cancer.
PLAN HIGHLIGHTS

Members and spouses are eligible for coverage regardless of age if
you have not been medically treated for or advised of cancer (except
skin cancer) within the five years prior to your effective date of
coverage.

Dependent unmarried children under age 19 are also eligible, 25 if
an enrolled Full-Time Student in an accredited college, university,
vocational or technical school (in Utah up to age 26).

Two options to choose from: Standard and High.

Standard Option pays $1,000 to use any way you wish upon first occurrence
(except skin cancer)...High Option pays $1,500.

Standard Option pays $100 a day for first 60 days of hospitalization,
$250 a day thereafter...High Option pays $150 a day and $375 a day
respectively.

Standard Option pays $100 a day for Outpatient Treatment...High Option
pays $150 a day.

Standard Option pays $100 per day for a maximum of 180 days up to
$18,000 for Hospice Care...High Option pays $150 a day up to $27,000.
- Intensive Care Unit Benefit of $100 per day (Standard Option) and
$150 per day (High Option) for a maximum of 15 days.
