Dental Insurance

The freedom you want to choose the dentists you trust!

  • Available as a separate plan if your firm has two or more employees OR can be added to the health insurance plan as an additional benefit with five or more employees

  • Benefits vary, but a typical plan offers a low $50 deductible, 100% preventive care, 75% basic care, and 50% major care benefits up to an annual maximum of $1,000 per covered person per calendar year

Your choice of three coverage types:

Innovative "Open Access" HMO
Lower premiums than traditional coverage...fewer restrictions than HMO's

  • No primary care physician required
  • Access to a large local network of participating providers
  • No referral needed to see a specialist
  • Lower out-of-pocket costs for in-network services
  • Includes prescription drugs, preventive care, well-baby care, worldwide emergency room care, and optional non-network benefits

Point of Service

  • Primary care physician required
  • A large network of participating providers
  • Low co-pays for in-network services
  • Out-of-network benefits included

PPO and Managed Indemnity (Nationwide network)

  • No primary care physician required
  • Benefits with a network provider: lower out-of-pocket expenses, no balance billing, no complicated claim forms to complete or track

 

Term Life Insurance with Accidental Death & Dismemberment

  • Available with or without health insurance to groups of two or more employees
  • Up to $100,000 of coverage per employee ($50,000 if included on health insurance plan)
  • Dependent coverage also available

 

Short-Term Disability

Help prepare for accidents and the unexpected!

  • Benefits paid on the first day of an accident or the 8th day of an illness, for benefit period options of 13 or 26 weeks, with a maximum weekly benefit of $750
  • Available without health insurance to groups with two or more employees (not available with health insurance plan)

 

Long-Term Disability

To help protect one of your most valuable assets-your income

  • Available to groups with 10 or more employees
  • Benefits paid at 60% of salary up to a maximum of $6,000 a month to age 65
  • Features an "Own Occupation" disability option
